Welcome to the Client Portal

The client portal is where you complete your initial intake forms, and review and print out your statements should you need them.

 

If this is your first time here, you should have received an email from us with your username and password. You can enter that by clicking the "Sign In" button on the top right of the page. If you have not yet received your login details, please email admin@eddinscounseling.com and request your userid and login.

 

Please note that we do not use the appointment scheduler in this portal. To schedule, change or cancel an appointment, you can visit our online scheduler found on our website http://www.eddinscounseling.com (at the top under "book an appointment").

 

To return to this portal, look for the "Patient Login" link on the bottom left of our website homepage.

 

IF YOU ARE A NEW CLIENT, please complete the initial paperwork in this client portal 24 hours prior to your first appointment. Once logged in, you will see the list of documents to complete.

PASSWORD PROTECTION INFO: The information you submit is secured and no one else has access to your clinical information as it is password protected. Once logged in, you will have the option of changing your password by clicking on change password on the left top portion of the screen. In order to get back to the main page, click the back button at the bottom of each form or simply click on "Home" from the top menu bar.

FOR CLIENTS UNDER 18: a parent or guardian will need to electronically sign your consent forms. Please be sure your parent is available to read and sign them prior to your initial appointment.
